By participating in these surveys, you will be adding important information to our research database. The more information researchers have, the more accurate the data will be when analyzing myeloma patients, their health status, and outcomes.

Below are seven health surveys and details about each. You can select one or more of these surveys to contribute to your patient database.

How can I take these surveys?

• Description: This is a HealthTree survey about your current health status. Your health condition changes over time, so please always check for the latest information.
• Questions include: This survey asks about your heart, lungs, bones, kidneys, diabetes, blood pressure, blood clots (DVT), neurological disorders, infections, and anxiety conditions. You will also be asked about your ability to walk and exercise, shop for groceries, or prepare meals without assistance, whether you have a caregiver, your eligibility for a stem cell transplant, and your current myeloma status.
• Time: This survey will take approximately 6 minutes.

• Description: HealthTree survey of cardiovascular and pulmonary health history.
• Questions asked: The survey asks about a history of cardiovascular disease, including high blood pressure, abnormal heart rhythms or arrhythmias, deep vein thrombosis (DVT), stroke, pulmonary embolism, heart failure, myocardial infarction, coronary artery disease, and peripheral artery disease. • Questions asked: This survey asks about your family’s history of the following cancers: bladder, breast, brain, colon, colorectal, Hodgkins disease, kidney, leukemia, liver, lung, lymphoma, melanoma, mouth/throat, Myeloma, smoldering myeloma, MGUS, pancreatic, prostate, rectal, sarcoma, stomach, thyroid, uterus/endometrium, unknown, other, cervix, or ovary.
• Time: Depending on your answers and your knowledge of your family history of cancer, this survey may take up to 10 minutes.

• Description: HealthTree research on blood type, allergies, autoimmune conditions, or chronic infections.
• Questions include: This survey asks you to identify your blood type, collagen disease, ulcer disease, food, drug, insect, latex, mold, pet, pollen allergies, and whether you have been diagnosed with an autoimmune disease such as AIDS . , autoimmune anemia, cerebrovascular disease, Hashimoto’s thyroiditis, Graves’ disease, psoriasis, lupus, inflammatory bowel disease, Raynaud’s syndrome, scleroderma, Sjögren’s syndrome, MS (multiple sclerosis), fibromyalgia, or Another autoimmune disease. • Description: HealthTree research on related health conditions such as diabetes, thyroid, kidney, gastrointestinal, neurological, and musculoskeletal problems (including osteonecrosis of the jaw).
• Questions include: This survey asks if you have ever been diagnosed with diabetes, hyperlipidemia, thyroid disease, or kidney, gastrointestinal, neurological, or musculoskeletal disease.
• Time: Depending on your responses, this survey may take up to 15 minutes.

• Description: HealthTree research on past surgeries, other cancer diagnoses, and other related mental health concerns.
• Questions asked: This survey asks if you have had any of the following surgeries: appendectomy, cholecystectomy, hysterectomy, hernia repair, mastectomy, caesarean section, prostatectomy, cataract. removal surgery, joint replacement, fracture repair, heart bypass surgery, stent procedure, angioplasty, or other surgery. We also ask about cancer-related diagnoses other than myeloma, such as basal cell carcinoma or other cancers listed above. Finally, ask about mental health disorders such as depression, anxiety/panic attacks, PTSD, insomnia, and other mental health conditions.
• Time: Depending on your responses, this survey may take up to 30 minutes.
