By participating in these surveys, you will be adding important information to our research database. The more information researchers have, the more accurate the data will be when analyzing myeloma patients, their health status, and outcomes.
Below are seven health surveys and details about each. You can select one or more of these surveys to contribute to your patient database.

How can I take these surveys?

- Description: This is a HealthTree survey about your current health status. Your health condition changes over time, so please always check for the latest information.
- Questions include: This survey asks about your heart, lungs, bones, kidneys, diabetes, blood pressure, blood clots (DVT), neurological disorders, infections, and anxiety conditions. You will also be asked about your ability to walk and exercise, shop for groceries, or prepare meals without assistance, whether you have a caregiver, your eligibility for a stem cell transplant, and your current myeloma status.
- Time: This survey will take approximately 6 minutes.
- Description: HealthTree survey of cardiovascular and pulmonary health history.
- Questions asked: The survey asks about a history of cardiovascular disease, including high blood pressure, abnormal heart rhythms or arrhythmias, deep vein thrombosis (DVT), stroke, pulmonary embolism, heart failure, myocardial infarction, coronary artery disease, and peripheral artery disease. - Time: Depending on your answers and your knowledge of your family history of cancer, this survey may take up to 10 minutes.
- Description: HealthTree research on blood type, allergies, autoimmune conditions, or chronic infections.
